Require a health questionnaire before the first booking
Ask clients to complete a health form once, store it on their profile, and show it to trainers at check-in.
Before you start

Open Studio → Business Settings → Booking
Go to Studio, open Business Settings, and switch to the Booking tab. The health questionnaire sits with your other public-booking options.

Turn the questionnaire on
Enable Require before first booking. It is off by default, so existing studios are not gated until you switch it on.

What clients fill in
Before their first booking or waitlist join, clients complete a short form: medical conditions or injuries, medications if relevant, an emergency contact (name, phone, relationship), and consent that the information is accurate.

It stays on the client profile
The answers are stored on that client's profile for your studio only. Clients can update them later in their account under Settings. Other studios the client visits do not see this form.

What trainers see at check-in
On the class participant list, the dashboard check-in card, and the client profile, trainers see a badge: the form is missing, health notes if conditions or medications were given, or the completed form with emergency contact. Tap the badge to expand the details.
Tips
- Staff can still add walk-in bookings. If the form is missing, trainers see that at check-in.
- Manual medical notes on the client profile remain available for studio-specific remarks.
